Statins and Cholesterol: It’s How You Lower It

Your cholesterol went up. Did anyone ask why?

A few weeks ago I watched a cardiologist in her mid-thirties tell her followers she had started a statin. The statin, she said, was for her future self.

No trial has ever tested whether a statin prevents a single heart attack or death in healthy adults under 40 who don't have a genetic cholesterol disorder. The people in the big statin trials averaged 63 years old, and the trials ran about four years. And nobody in that story asked the question this whole episode is built on: why did the number go up in the first place?

Cholesterol is raw material

Most people picture cholesterol as gunk in a pipe, when your body treats it as raw material. Every cell wall is built with it, and every steroid hormone you make starts from it: progesterone, DHEA, testosterone, estradiol and cortisol.

Lower isn't automatically better, either. In about 70 of roughly 95 cohort studies, people whose cholesterol ran naturally low died at higher rates than people in the middle. Illness pulls cholesterol down too, so part of that runs the other way, but your body still doesn't treat cholesterol as trash.

Why the number climbs in midlife

In women, LDL climbs on the same schedule hormones fall. When researchers lined women's cholesterol up against their own final period, LDL rose more than three times faster in the couple of years around it than before it, independent of age, and after it the climb nearly stopped. Estradiol helps your liver keep the receptors that pull LDL out of the blood. When estradiol leaves, clearance slows.

Thyroid is the strongest lever in this whole body of research. When people whose thyroid has been removed come off their thyroid hormone for a few weeks, their LDL runs about 70 points higher than once they're back on it. In 2015 an international lipid panel, 13 of its 31 members tied to the companies that make cholesterol drugs, wrote that thyroid should be checked before any statin starts.

In men, lower testosterone travels with the metabolic syndrome, and underneath all of it sits insulin resistance. Anything that improves insulin sensitivity is a cholesterol treatment, whether or not anybody calls it one.

What a statin actually does

A statin blocks one step early in the liver's factory line, and that step feeds much more than cholesterol. It also supplies coenzyme Q10, which your mitochondria use to make energy, and below cholesterol sit your steroid hormones. Picture a dam near the source of a river. The cholesterol town runs dry, which is the point, and some of the other towns get less water too: CoQ10 falls, and so does testosterone.

In a 2013 trial, sedentary adults carrying extra weight trained for 12 weeks. Without a statin their fitness rose about 10%, and with one, about 1.5%. The energy-making machinery in their muscles grew about 13% without the drug and didn't grow with it.

A statin lowers the number without touching why it rose. It also changes your calcium score: in the words of a 2015 analysis of the plaque-imaging trials, statins promote calcium in the plaque. So a score taken on a statin can't be read the same way as one taken before it.

The real numbers

"38% fewer heart attacks" sounds enormous. In people without heart disease, it comes to about 7 fewer heart attacks for every 1,000 people taking a statin for four to five years. Of ten pooled estimates, six found no significant difference in deaths from any cause, though all ten leaned toward fewer, and within the trials the extra life averaged out to about six to ten days.

After a heart attack, or with established heart and artery disease, the numbers change. That is where statins are at their strongest: about two fewer heart attacks per 100 people over four to five years, and deaths from any cause about 17% lower, about two fewer per 100 over five years. If that's you, those numbers belong in a conversation with your cardiologist, and your hormones, thyroid and insulin still matter alongside them.

The price

The biggest cost is blood sugar. On a high-dose statin, about 1 extra person in 80 is diagnosed with diabetes each year, and about six in ten of those extra cases land in people whose blood sugar was already in the top quarter. The people closest to the line get pushed over it, and that is often the insulin-resistant patient whose cholesterol rose in the first place.

Statins also lower testosterone in men, more at higher doses. In men who were already low, switching to a high-dose statin took another 15 to 19% off in four months. For a man just above the line the FDA uses to call it low, that's enough to put him under it, for as long as he takes the drug. Serious muscle damage is rare and tied to dose and genes. Milder muscle pain is real for some people, and only one of 42 statin trials specifically asked about it.

Hormones push the other way. Across randomized trials in women past menopause, new diabetes ran about 30% lower on hormone therapy.

How you lower it

It can be the same lower number on the panel with a very different body underneath. Oral estradiol lowers LDL, lowers ApoB and lowers Lp(a), which very little else moves. Taken alone or with progesterone, it raises HDL. Triglycerides tick up modestly at first, and in my experience that settles. The patch lowers LDL more gently, and it's a good option and her choice. For men, I add oral estradiol on top of testosterone, because estradiol is what moves ApoB down and ApoA up. And when thyroid is truly low, treating it is the biggest single cholesterol move in the research.

The numbers I watch hardest aren't LDL. My targets are an ApoB under 90, an ApoA-1 over 125, an HDL over 60 and triglycerides under 50, optimal, not just normal. In women I want estradiol at least 75, a floor, not a ceiling.

Questions for your next appointment

What is my absolute risk, out of 100 people like me over five years, and how much does this drug change it? Is this to prevent a first heart attack or a second one? Have my thyroid and my hormones been checked? What does my calcium score show, and was it taken before the statin?

Then you decide, with the person who prescribes it. Nobody should stop a prescribed medicine because of an article; this is a reason to walk in with better questions than the prescriber expected. And if your doctor gets upset by these questions, it's time to find a new doctor.
