Normal Testosterone for Your Age: What the Number Means
Three hundred started life as a rule for entering a clinical trial. Here is where the number came from, what a low level costs a man, and why waiting is not free.
He used to be the guy who got up first. He is 46, he runs a crew, and about three years ago that stopped. He is tired in a way sleep does not fix, he has put on twenty pounds eating the same, and he says he is about seventy percent of himself.
So he asks for his testosterone. It comes back at 310, he is told that is normal for his age, and he walks out with an antidepressant, a pill for the bedroom, and a suggestion that he lose some weight.
Measured against healthy men in their twenties and thirties, 310 is about the bottom five percent.
Where the number 300 came from
It is not a law of anything. It was the cutoff the FDA picked to decide which men could join the trials of the first testosterone products, an entry criterion first, a diagnosis afterwards. The Endocrine Society adopted it in 2006, withdrew it in 2010, and it survived.
The bodies that publish these thresholds do not agree: the Endocrine Society now uses 264, the American urologists 300, the European association 348. The same man, with the same blood, is deficient in Europe and normal here.
The laboratory is worse. Twenty five labs asked for their normal range gave seventeen different answers, with printed floors from 130 to 450. The ranges came from the assay manufacturers, and not one had validated them.
That is what a reference range is: the middle ninety five percent of whoever happened to get tested at that lab, and sick men are in that pile, because sick men get their blood drawn. It never was a measurement of health.
The yardstick that does mean something
Researchers measured 1,185 healthy, non obese men aged 19 to 39 by mass spectrometry against one harmonised standard, and published what a healthy young man runs. The median was 531. The bottom five percent started at 303.
So the man at 310 sits at the fifth percentile of healthy young men, ninety five percent of them above him, and he feels like seventy percent of himself. Those two facts are the same fact.
There is also a better number than the total. Most testosterone in the blood is bound to carrier proteins, and the free fraction does the work. In over 3,000 European men, a normal total with a low free testosterone was symptomatic; a low total with a normal free was almost not. When the carrier protein is low the free level reads high, so the two get read together.
And that yardstick has been shrinking for forty years
Measured in the same population across three waves, a 65 year old in 2003 had less testosterone than one in 1988, about one percent less per calendar year, after adjustment for obesity, smoking and illness.
Then it reached the young men. In 4,000 American men aged 15 to 39, average total testosterone fell from 605 to 451 between 1999 and 2016, and the share below 300 went from four percent to about twenty. In that age band a man's age did not predict his level. The year he was measured did. In a separate analysis of more than a million men, body mass index did not trend in the 700,000 it had the numbers for, so this is not his waistline.
Young men are the reference population. When they fall, the range follows them down.
What a low level costs him
It is a quality of life problem and a disease problem. A trial called T4DM put 1,007 overweight men with prediabetes or new type 2 diabetes through the same two year weight loss programme and gave half of them testosterone. The control condition is exactly the advice this man was sent home with.
At two years, 87 of 413 men on the programme alone had type 2 diabetes. Among the men who also got testosterone, 55 of 443. Roughly one man in eleven.
Look at what came off. Both groups lost weight. The testosterone group lost almost 3 kg more fat, 2 cm more waist, and held their muscle. The programme alone cost the other men 1.3 kg of it.
Mortality was measured in 24,000 men with 7,000 deaths. Below about 213, all cause mortality rose, and below about 150, cardiovascular death rose. That is a threshold rather than a slope, and the man at 310 is closer to that line than anyone told him. The removal experiment runs the same way. In 73,000 men with prostate cancer, a third of them put on androgen deprivation, diabetes went from about 21 to 29 cases per thousand men per year.
What comes back, and how long it takes to see it
The scale is the wrong instrument early, and that is why men quit. In obese men with severe sleep apnoea, eighteen weeks of testosterone did not move the scale further than placebo, while muscle rose by 1.6 kg, liver fat fell, and arteries got measurably less stiff.
Over years it does move. A registry followed men for five years and watched weight fall from about 106 kg to 90, and waists come in from 107 cm to 98. Not randomised: the men chose whether to be treated. Argue muscle, waist and years.
Why waiting is not free
The same registry had an untreated comparison group. Over eight years those men gained about 8 kg and 7 cm of waist, and forty percent went on to type 2 diabetes.
In 83,000 veterans with low testosterone, men treated but never brought to a normal level had the same heart attack and stroke rates as men never treated. Being on the prescription is not the same as being treated.
The three prescriptions, and the fears
All three act downstream of the thing nobody measured. Among men whose depression had already failed an adequate course of antidepressants, 24 of 56 had a testosterone at 350 or below. The pill for the bedroom protects a chemical signal that testosterone is what keeps his body making. Weight loss is good for him, and the amount that would move the hormone is more than most men can hold.
Then come the fears: the prostate, the heart, thick blood, the temper, sleep apnoea, the children he might still want. Almost every one traces back to a source far smaller than the fear, and all of it is taken apart in the video and the two part podcast.
What to ask for
Ask for a total and a free testosterone with sex hormone binding globulin, not a total alone. Ask which reference range the lab used and where it came from. Ask to be compared against healthy young men. And ask whether you are being treated for your symptoms or your printout.
If he takes one sentence into his next appointment, it is this. I am not asking to be normal, I am asking to be well.
